Care Navigator Specialist / Intake Specialist


Compensation: $30–$40/hr (based on experience)

Schedule: Full-time | Remote

Location: Metro Denver residents only

Start Date: Mid-December


About the Role

If you’re someone who finds purpose in helping families through important life transitions, this role may be a wonderful fit. As the Senior Living Navigator, you’ll be the warm, reassuring first point of contact for families reaching out to Oasis Senior Advisors.


Your conversations and guidance help families feel informed, supported, and cared for—often during one of the most emotional decisions they will make. This is meaningful work, and we’re looking for someone who approaches it with compassion, professionalism, and a genuine desire to help.


This hybrid position is ideal for someone with 3+ years of experience in senior living, hospice intake, home care coordination, case management, or healthcare-related customer service. You will play a key role in gathering information, coordinating next steps, and supporting our Senior Living Advisors so they can maximize time with families in the field.


Key Responsibilities

Primary Responsibilities

● Represent Oasis Senior Advisors with warmth, empathy, and professionalism while upholding our Five R’s core values.

● Lead client/family intakes, gathering clinical, social, and logistical information with accuracy and clarity.

● Enter all intake details into IQ promptly and ensure documentation is complete and organized.

● Assist in creating Oasis Senior Ready Resource sheets and informational tools to help guide families.

● Participate actively in weekly team meetings and calls.


Additional Responsibilities

● Serve as the first point of contact for incoming calls through the Oasis phone tree.

● Manage and prioritize incoming referrals from hospitals, skilled nursing facilities, home care agencies, and community partners.

● Monitor the referral inbox and respond to inquiries with attentiveness and professionalism.

● Triage Medicaid/low-income referrals and provide appropriate resources.

● Send personalized “Oasis is here to help” emails with supportive information.

● Assign families to the correct Senior Living Advisor and ensure a smooth, well-documented handoff.

● Send, track, and upload Referral Fee Disclosure Forms (RFDFs).

● Enter clinical details and initiate CLS setup for Senior Living Advisors.

● Write clear, concise 800-character client bios for distribution to communities.

● Initiate and manage the client pipeline; host or support pipeline meetings as needed.

● Maintain regular, compassionate communication with families throughout their transition.

● Follow up after move-in and request Google reviews from clients.

● Represent Oasis at virtual community events, association meetings, and senior living expos.

● Document all interactions, updates, and CRM activity with accuracy and attention to detail.

● Coordinate with teammates to ensure intake coverage during planned time off.


Required Qualifications

We’re looking for someone who brings both heart and expertise to this role. To be successful, you must have:


Experience

● 3+ years experience in at least one of the following:

○ Senior living industry (admissions, placement, sales, resident services)

○ Hospice intake or home care coordination

○ Healthcare office intake or care navigation

○ Social work or case management

○ Customer service in a healthcare-related environment


Skills

● Highly detail-oriented with excellent organizational skills

● Experience handling intakes, referrals, or complex scheduling

● Excellent communication, warm, clear, and empathetic

● Strong online CSR or customer service background

● Ability to support families through emotional conversations

● Tech-comfortable and able to use CRM/EMR systems with ease

● Collaborative, reliable, and team-oriented

● Compassionate, steady, and calm under pressure


Nice-to-Have

● CSA certification


Work Schedule

Monday–Friday, 8:00 AM – 5:00 PM Mountain Time (1-hour lunch)
